Indications

Duova Inhaler is used for: Maintenance treatment of Chronic obstructive pulmonary disease (COPD)

Adult Dose

Inhalation Chronic obstructive pulmonary disease Adult: As inhalation cap: 12 mcg/18 mcg bid via inhaler device. As metered-dose aerosol: 12 mcg/18 mcg bid.

Patient w/ narrow-angle glaucoma, prostatic hyperplasia/bladder neck obstruction, cystic fibrosis. History of MI (<6 mth), or unstable or life threatening cardiac arrhythmia w/in the past yr. Not intended for use as initial treatment for acute bronchospasm and as monotherapy for asthma. Moderate to severe renal impairment (CrCl ≤50 mL/min). Pregnancy and lactation. Patient w/ CV disease, CNS disorders, hyperthyroidism, hypokalaemia, DM, phaeochromocytoma, seizure disorder. Patient Counselling This drug may cause dizziness and blurred vision, if affected, do not drive or operate machinery. Monitoring Parameters Monitor pulmonary function.

Adverse Effects

Side effects of Formoterol + Tiotropium : The most common anticholinergic adverse reaction reported by COPD patients was dry mouth, which was mild and often resolved during continued treatment in the majority of cases. In general, dry mouth had an onset of 3 and 5 weeks, which resolved while patients continued to receive tiotropium bromide. Serious undesirable effects consistent with anticholinergic effects include glaucoma (new onset or worsening), blurred vision, constipation, tachycardia, intestinal obstruction including ileus paralytic dysuria, and urinary retention. Viral infection, bronchitis, chest infection, dyspnoea, chest pain, tremor, dizziness, insomnia, tonsillitis, rash, dysphonia, serious asthma exacerbation. COPD: Upper resp tract infection, back pain, pharyngitis, chest pain, sinusitis, fever, leg cramps, muscle cramps, anxiety, pruritus, increased sputum, dry mouth. Potentially Fatal: Paradoxical bronchospasm.

Mechanism of Action

Formoterol stimulates intracellular adenyl cyclase, the enzyme that catalyses the conversion of ATP to cyclic-3', 5'-adenosine monophosphate (cAMP) resulting in relaxation of bronchial smooth muscle and inhibition of release of mediators of immediate hypersensitivity from mast cells. Tiotropium bromide antagonises the cholinergic effects of acetylcholine by reversibly and competitively binding to type 3 muscarinic (M3) receptors, resulting in bronchial smooth muscle relaxation.
