Indications

Econex Tablet is used for: Tuberculosis

Adult Dose

Oral Adult 800 mg ethambutol and 300 mg isoniazid, as a single daily dose Ethambutol and isoniazid in a fixed-dose combination should not be used in the initial phase of tuberculosis treatment {01}. Ethambutol and isoniazid in a fixed-dose combination can be substituted for thioacetazone and isoniazid combination in patients with side effects due to thioacetazone. Full course of therapy, which may take 6 months to 2 years.

Child Dose

Child < 14 years of age: Safety & efficacy has not been established.

Renal Dose

Patients with impaired renal function generally do not require a reduction in dose if the plasma creatinine concentration is less than 6 mg per 100 mL. If renal impairment is more severe or if patients are slow acetylators, a reduction in dose and/or serum determinations may be required.

Administration

Should be taken on an empty stomach. Best taken on an empty stomach 1 hr before or 2 hr after meals. May be taken w/ meals to reduce GI discomfort.

Contra Indications

Hypersensitivity. Patient w/ acute liver disease or a history of isoniazid-associated hepatic injury. Patient w/ known optic neuritis and poor vision.

Precautions

Patient w/ convulsive disorders, history of psychosis. Patient at risk of neuropathy (e.g. diabetics, alcoholics, malnourished, uraemic, infected w/ HIV) or pyridoxine deficiency. Patient w/ ocular defects (e.g. cataracts, recurrent ocular inflammatory conditions, diabetic neuropathy). Hepatic and severe renal impairment. Pregnancy and lactation. Monitoring Parameters Monitor baseline and periodic LFTs (ALT and AST), sputum cultures mthly until 2 consecutive negative cultures reported and prodromal signs of hepatitis.

Interactions

Inhibit the hepatic metabolism of antiepileptics (e.g. carbamazepine, ethosuximide, primidone, phenytoin), benzodiazepines (e.g. diazepam, triazolam), chlorzoxazone, theophylline, disulfiram, sometimes leading to increased toxicity. Increased metabolism of enflurane, resulting in potentially nephrotoxic levels of fluoride. Increased concentrations and enhanced effects or toxicity of clofazimine, cycloserine and warfarin. Reduced absorption w/ Al-containing antacids. Increased risk of peripheral neuropathy w/ zalcitabine and stavudine.

Adverse Effects

Side effects of Isoniazid + Ethambutol : Peripheral neuritis, psychotic reactions, convulsions, optic neuritis, transient increases in liver enzymes; haematological effects (e.g. anaemias, agranulocytosis, thrombocytopenia, eosinophilia); hypersensitivity reactions include skin eruptions (including erythema multiforme), fever, vasculitis; nausea, vomiting, dry mouth, constipation, pellagra, purpura, hyperglycaemia, lupus-like syndrome, vertigo, hyperreflexia, urinary retention, gynaecomastia. Retrobulbar neuritis w/ reduction in visual acuity, constriction of visual field, central or peripheral scotoma and green-red colour blindness of 1 or both eyes. Potentially Fatal: Hepatitis.

Mechanism of Action

Isoniazid inhibits the synthesis of mycoloic acids in susceptible bacteria which results in loss of acid-fastness and disruption of bacterial cell wall. At therapeutic levels, it is bacteriocidal against actively growing intracellular and extracellular Mycobacterium tuberculosis organisms. Ethambutol appears to inhibit the synthesis of 1 or more metabolites in susceptible bacteria resulting in impairment of cellular metabolism, arrest of multiplication, and cell death. It is active against susceptible bacteria only when they are undergoing cell division.
