Indications
Kaf D Syrup is used for:
Cough and common cold
Adult Dose
Oral
Cough and common cold
Adult: Per 5 ml syrup contains triprolidine HCl 1.25 mg, phenylephrine 5 mg and dextromethorphan HBr 10 mg: 10 ml 4 -6 hrly, up to 4 times daily;
Child Dose
Oral
Cough and common cold
Child: Per 5 ml syrup contains triprolidine HCl 1.25 mg, phenylephrine 5 mg and dextromethorphan HBr 10 mg:
>12 yr: 10 ml 4 -6 hrly, up to 4 times daily;
6-12 yr: 5 ml 4 -6 hrly, up 4 times daily;
2-5 yr: 2.5 ml 4 - 6 hrly, up to 4 times daily

Contra Indications
Severe hypertension or coronary artery disease. Patient at risk of respiratory failure. Avoid use in patients where cough is associated with asthma or with excessive secretion. Concurrent use or within 2 wk of stopping MAOIs.
Precautions
Severe hepatic impairment, severe or moderate renal impairment. Hypertension, heart disease, DM, hyperthyroidism, increased intra-ocular pressure, prostatic hypertrophy. May impair ability to drive or operate machinery. Pregnancy and lactation.
Pregnancy-Lactation
Interactions
Increased BP with other sympathomimetic agents e.g. decongestants, amphetamine-like psychostimulants. May reduce hypotensive effect of bretylium, betanidine, guanethidine, debrisoquine, methyldopa, α- and β- blockers.
Potentially Fatal: Increased risk of hypertensive crisis with furazolidine; avoid concurrent use. Increased risk of hypertensive crisis with MAOIs; avoid concurrent use or within 2 wk after stopping MAOIs.
Adverse Effects
Side effects of Dextromethorphan + Phenylephrine + Triprolidine :
Nausea, vomiting, dry mouth, nose and throat, GI disturbances, skin rashes, drowsiness, sleep disturbance, tachycardia, urinary retention. Rarely, hallucinations.
Mechanism of Action
Triprolidine is a potent competitive histamine H1-receptor antagonist with mild sedating and antimuscarinic properties.
Phenylephrine is a vasoconstrictor and a decongestant that relieves symptoms resulting from irritation of upper respiratory tract tissue. It shrinks swollen mucous membranes, reduces nasal congestion and tissue hyperemia.
Dextromethorphan is a centrally acting cough suppressant which acts on the cough centre in the medulla.
