Indications
Respira P Expectorant Liquid is used for:
For the relief of bronchospasm in asthma & allied conditions, chronic bronchitis & emphysema.
Adult Dose
Adult 10 mL (2 tsp) tid.
Child Dose
Childn 6-12 yr 5-10 mL (1-2 tsp) tid
Childn <6 yr 5 mL (1 tsp) tid.
Renal Dose
Administration
Should be taken with food.
Contra Indications
Hypersensitivity. Patients w/ pre-existing cardiac tachyarrhythmias & thyrotoxicosis.
Precautions
Patients w/ DM, serious CV disorders, HTN, hyperthyroidism & peptic ulcer, history of peptic ulceration. Fine finger tremors, palpitations & GI disturbances.
Pregnancy-Lactation
Interactions
Increased pressure response w/ MAOI's. Increased efficacy w/ other sympathomimetics. May reduce action w/ β-blocker. Increased risk of hypocalcemia w/ theophylline. Decreased serum w/ digoxin levels.
Adverse Effects
Side effects of Salbutamol + Bromhexine + Guaifenesin :
Nausea & vomiting. Bromhexine: Risk of allergic reactions; sever skin reactions (SCARs): Erythema multiforme, SJS. Salbutamol: Tremors. Headache; tachycardia, palpitations; muscle cramps.
Mechanism of Action
Salbutamol activates adenyl cyclase, the enzyme that stimulates the production of cyclic adenosine-3', 5'-monophosphate (cAMP). Increased cAMP leads to activation of protein kinase A, which inhibits phosphorylation of myosin and lowers intracellular ionic Ca concentrations, resulting in smooth muscle relaxation.
Bromhexine enhances mucus transport by reducing mucus viscosity and by activating the ciliated epithelium (mucociliary clearance). This secretolytic and secretomotor effect in the bronchial tract area relieves cough and facilitates expectoration.
Guaifenesin acts as an expectorant by increasing the effective hydration of the respiratory gland thereby increasing the volume and reducing the viscosity of tenacious bronchial secretions, thus facilitating its removal by natural clearance processes.
