Indications

Gentamicin + Dexamethasone Ear prep is used for: Otitis externa

Adult Dose

Otic/Aural Otitis externa Adult: Ear drops: Instill 2-3 drops in the affected ear 3-4 times a day and at night.

Child Dose

Otic/Aural Otitis externa Child: Instill 2-3 drops in the affected ear 3-4 times a day and at night.

Contra Indications

Hypersensitivity to gentamicin and other aminoglycosides. Topical application into the ear for patients w/ known or suspected perforation of ear drum.

Precautions

Monitoring Parameters Monitor renal, hepatic, vestibular and cochlear function.

Adverse Effects

Side effects of Gentamicin + Dexamethasone Ear prep : Ototoxicity, electrolyte disturbances (notably hypomagnesaemia, but also hypocalcaemia and hypokalaemia);

Mechanism of Action

Gentamicin interferes w/ bacterial protein synthesis by binding to 30S and 50S ribosomal subunits resulting in a defective bacterial cell membrane. Dexamethasone decreases inflammation by suppressing migration of polymorphonuclear leukocytes (PMNs) and reducing capillary permeability; stabilizes cell and lysosomal membranes, increases surfactant synthesis, increases serum vitamin A concentration, and inhibits prostaglandin and proinflammatory cytokines; suppresses lymphocyte proliferation through direct cytolysis, inhibits mitosis, breaks down granulocyte aggregates, and improves pulmonary microcirculation
